How to order Personalized Dog Tags Online:

1. Purchase a set of Personalized Dog Tags through the Web Store.

2. Click or Download (Right Click + Save As) the Dog Tag Form from the link above.

3. Complete the form by filling in the necessary fields in the PDF file.

4. Save a copy of your file on your computer.

5. Email an electronic copy of your completed form to us at info@supplysergeant.ca

OR

Print your completed form and fax it to us at 780-486-3422.

6. Don't forget to include your Order Number (found in the email receipt of your webstore purchase) on your Dog Tag Form.

7. Wait for your awesome personalized Dog Tags to arrive.
